Disclosure: This article is written by the PsychDesk team, and PsychDesk is one of the apps reviewed. We've tried to describe every competitor fairly, using their own published pricing and feature pages as of September 2026. Pricing changes often, so check each vendor's site before you decide.

A practice management app for therapists is software that handles the admin side of running a practice: booking sessions, writing clinical notes, sending invoices, and keeping client records in one place. The best one for you depends less on feature count and more on where you practise, how you get paid, and whether you work alone or with a team.

Most "best therapy app" lists are written for American practices, where insurance billing decides everything. This one is written for therapists in India, where most clients pay directly, UPI is the default, and the data law that applies to you is the DPDP Act, not HIPAA.

How we evaluated these apps

We looked at five things that matter most to a practice in India:

  1. Payments that match how clients pay. UPI support and INR billing, not just an international card gateway.

  2. Clinical documentation. Whether notes are structured (SOAP, DAP), whether AI help exists, and whether you review before anything saves.

  3. Data protection. Encryption, access control, and whether the app speaks to India's DPDP Act rather than only US rules.

  4. Room to grow. What happens when you hire an associate or open a second location.

  5. Real cost. The price of the plan you'd actually need, not just the cheapest number on the page.

1. PsychDesk

Best for: therapists and clinics in India who want scheduling, notes, billing and records connected in one place, and who expect to grow.

PsychDesk is a practice management and EHR platform built for how mental health professionals in India work. A booked session carries its note, its invoice and the client's history together, so nothing lives in three separate places.

Key features

  • AI-assisted notes: turn a few shorthand lines into a SOAP or DAP draft in your own writing style. You edit and approve every note before it saves.

  • Smart scheduling: client self-booking links with automatic SMS and email reminders.

  • Client records: one encrypted chart per client with sessions, documents and history.

  • Billing: one-click invoices in INR, card and UPI payments, GST-compliant invoicing, and automatic payment nudges.

  • Therapy worksheets: assign CBT tools and mood logs between sessions. Responses sync back to the client's chart.

  • Clinic tools: multi-branch management, team roles and permissions, and advanced reporting on the Clinic+ plan.

  • Compliance: designed around the DPDP Act, with HIPAA-aligned safeguards and AES-256 encryption at rest.

Pricing

  • Basic: ₹799/month (up to 10 clients, 1 therapist seat)

  • Standard: contact sales (up to 50 clients, 3 therapist seats, client self-service portal)

  • Clinic+: contact sales (unlimited clients and seats, multi-branch, dedicated account manager, API access)

  • 30-day free trial, no credit card required

Where it falls short

  • No permanent free plan. PractiPal and Cognixa both offer one for very small caseloads.

  • Standard and Clinic+ pricing isn't published, so larger practices need a sales conversation to get a number.

  • Not built for US insurance claim billing. If that's your main need, look at SimplePractice.

2. PractiPal

Best for: therapists just starting out who want a free, friendly tool for a small caseload.

PractiPal is an India-built practice management app with a warm, community-focused feel. Its free plan makes it an easy first step for someone moving off spreadsheets and WhatsApp.

Key features

  • Scheduling with Google Calendar sync on paid plans

  • Invoicing and a financial dashboard for tracking pending payments

  • Secure notes and a resource bank for sharing material with clients

  • A one-page therapist profile

  • Priority WhatsApp support on paid plans

Pricing (from PractiPal's site, September 2026)

  • Free: up to 5 clients, 1GB storage, UPI payments

  • Growing Ensemble: ₹1,499/month, unlimited clients, 5GB storage

  • Therapy Empire: ₹1,899/month plus ₹1,000 per additional therapist (listed as "coming soon" at the time of writing)

  • 14-day free trial on paid plans

Where it falls short

  • Multi-therapist features were still marked "coming soon" when we checked, so group practices may need to wait.

  • AI-assisted clinical notes aren't advertised as a feature.

  • Integrated payment gateways (Razorpay, PayPal) are an add-on with an extra fee.

3. Cognixa

Best for: psychologists who want AI help with notes and care a lot about exactly how their data is handled.

Cognixa is an India-built, AI-assisted practice management app for psychologists. It stands out for how specifically it explains its security.

Key features

  • AI-assisted notes, with the option to write your own way

  • UPI-based invoicing and payment tracking

  • Customisable timings, note formats, intake forms and session styles

  • Clinical fields encrypted with AES-256-GCM; temporary AI note data removed after 30 minutes

  • HIPAA-aligned safeguards and DPDP-ready privacy controls

Pricing

  • Free forever for up to 5 clients

  • Paid plan pricing wasn't published on its homepage at the time of writing

Where it falls short

  • Unclear pricing beyond the free plan makes cost planning harder.

  • Less visible support for group practices and multi-location clinics.

4. PracFlow

Best for: therapists who want their practice tools, a website and a referral network in one subscription.

PracFlow is an India-focused practice management app that bundles marketing tools alongside the admin side.

Key features

  • Encrypted case notes, intake forms and client assignments

  • AI-powered session summaries

  • Private client chat and WhatsApp integration

  • A built-in therapist website

  • A therapist-to-therapist referral system

  • UPI payments and INR pricing, with DPDP compliance

Pricing

  • Pay-as-you-go: from ₹0

  • Monthly: ₹1,199/month

Where it falls short

  • Bundling a website and referrals is useful if you need them, but it's extra surface area if you only want clean admin tools.

  • Clinic-scale features like per-branch reporting are less prominent than on platforms built for multi-location practices.

5. SimplePractice

Best for: practices that bill US insurance, see US-based clients, or need telehealth and e-prescribing in one US-centred system.

SimplePractice is one of the most established practice management platforms in the world, with deep insurance billing and a large feature set. It's the benchmark most other apps get compared to.

Key features

  • Scheduling, notes, billing and client portal

  • Strong insurance claim handling

  • Telehealth

  • Optional AI Note Taker and ePrescribe add-ons

Pricing (USD)

  • Starter: $49/month (excludes calendar sync and insurance claims)

  • Essential: $79/month

  • Plus: $99/month

  • Add-ons: AI Note Taker $35/month, ePrescribe $49/month

Where it falls short for Indian practices

  • Billed in USD, so your card adds a foreign transaction markup every month.

  • No UPI and no GST invoicing.

  • Built around HIPAA, which is US law. It doesn't address your obligations under India's DPDP Act.

  • The Starter price rose from $29 to $49 in 2025, and its reminders are limited on that plan.

Looking for a SimplePractice alternative in India?

Which app should you choose?

  • You're solo, under 5 clients, and want to spend nothing: PractiPal or Cognixa's free plans are a reasonable place to start.

  • You want a website and referral network bundled in: PracFlow.

  • You run a group practice or multi-location clinic: PsychDesk's Standard or Clinic+ plans are built for team roles, shared scheduling and per-branch reporting. See the clinic-specific guide

  • You bill US insurance or serve mainly US clients: SimplePractice.

FAQ

It depends on your size and needs. For most Indian practices, the right app supports UPI payments, GST invoicing and India's DPDP Act. PsychDesk, PractiPal, Cognixa and PracFlow are all built for India. SimplePractice suits practices billing US insurance.

Yes. PractiPal and Cognixa both offer free plans for up to 5 clients, and PracFlow has a pay-as-you-go option from ₹0. PsychDesk offers a 30-day free trial with no credit card, then starts at ₹799/month for up to 10 clients.

PsychDesk, Cognixa and PracFlow include AI help with notes or session summaries. SimplePractice offers an AI Note Taker as a $35/month add-on. With any of them, you should review AI drafts before they become part of the clinical record.

HIPAA is a US law and doesn't govern practices in India. The law that applies is the Digital Personal Data Protection Act, 2023. Look for an app that addresses DPDP obligations like consent, encryption and data export, not just a HIPAA badge.

Usually, yes. Check that your current app lets you export clients, notes and invoices in a standard format before you switch, and that the new app can import them.
